From my research I've created a prescription record leaflet with all the details and information from there eye test. I used the same type i had used for the posters to make them work as a set.
I changed the tick boxes to having the pattern as a fill with a low opacity to alter it from the usual formal style.
I also put the back ground the same with an even lower opacity
Front cover - using the same idea as the appointment card to keep theme within the products
